Financial Policy
We are now affiliated with Summa Physicians, Inc. and all billing for patient services will
be processed by Summa. If you have any billing questions related to dates of service
from July 1, 2010 to current please call 888-923-7028. For dates of service prior to
July 1, 2010 call 330-723-3256 option 3.
Your health insurance company and you have a contract that reimburses your
physician based upon compliance with their coverage benefits. It is critical to your
care that you are familiar with your benefits and all the restrictions, co-pays and
requirements of your insurance plan. As a service to you we will file to your primary
and secondary insurance plans.
INSURANCE COVERAGE
- Bring your current insurance card to every visit.
- Notify the office of any change of coverage ASAP
- To assure you are covered, consult your insurance company's physician guide
to make sure our physicians are with your plan to maximize your benefits
- Review all data and verify that the information for billing is correct, adding any
dependents that are covered
- Provide current insurance information in order for your claim to be paid.
SELF-PAY / MOTOR VEHICLE ACCIDENTS
- Payment in full is expected at the time of service
- Arrangements are available for hardship situations
WORKERS COMPENSATION
(FOR CURRENT PATIENTS ONLY, EFFECTIVE JULY 1, 2003)
- A FIRST REPORT OF INJURY must be filled out with your employer at the time
of the injury
- Have this information available when you arrive at your first appointment:
Employer name, phone number, human resource contact, employer MCO, date
of injury.
ALL BALANCES BILLED ARE DUE UPON RECEIPT, UNPAID BALANCES GREATER
THAN 30 DAYS ARE SUBJECT TO OUR COLLECTION PROCESS. WE ACCEPT
Debit Card/HSA, Discover, MasterCard and Visa.
