| Forms: Please complete patient portion prior to leaving it at the office. Forms will be filled out within 5-7 days, call before picking them up to be assured of their completion. Patient/Physician relationship: For continuity of care it is best to see the same physician at each visit if possible. Non-emergency appointments: Will be scheduled with your primary care physician. |
